Cousino Athletic Families,
Starting the 2025-2026 School Year, all of our Athletic Forms are now going to be online. You must have the following forms completed to be eligible to participate in Athletics @ Cousino HS:
- Current Sports Physical - On/After April 15, 2025
- Insurance Form
- Concussion Form
- WCS Code of Conduct
- Medical Authorization Form
- Emergency Contact Form
All accounts must be made via Student Central Account Creation. Step by Step directions are attached to this Announcement.
